How to Enroll in the Adult Day Program 

To enroll in the House of Welcome (HOW) Adult Day Program, please follow the steps below. Once we receive the application forms and the medical report from your primary care doctor, we will schedule an interview with our Clinical Manager/Caregiver Specialist.

The two enrollment forms are fillable PDFs. You can download them to your computer or print them and fill them out by hand. To save them to your computer, please add your family member’s name to the file name when you save it (for example, Demographics & Health History - Pat Jones). When they are complete, please email both files to HOW@nssc.org.

If you fill out the forms by hand, please print legibly. Forms may be mailed or dropped off at HOW, 1779 Winnetka Road, Northfield, IL, 60093, during business hours.

1. Complete the Demographics & Health History form.

2. Fill out the Personal History application.

3. Download the HOW Medical Report and ask your family member's primary care doctor to fax it to HOW at 847.242.6275.
