Support for Living Well With Dementia
What is GUIDE?
GUIDE (Guiding an Improved Dementia Experience) provides an additional layer of support for people living with dementia to help improve quality of life so they can live safely at home for as long as possible. Caregivers receive practical support, education, and 24/7 access to medical expertise. Some caregivers qualify for respite services. GUIDE is a free program through Medicare.
GUIDE is covered by Medicare. There are no costs for eligible participants.
What you get with GUIDE:
- Personalized care plan that adapts as needs evolve
- Comprehensive assessment to understand the specific challenges and strengths of each individual and caregiver
- Ongoing support with regular check-ins to help navigate the complexities of dementia care
24/7 acccess to medical expertise - Referral to caregiver services that offer support and education
- Free respite care (time off from caregiving) for those who qualify
Who’s eligible
- You have a dementia or dementia symptoms
- You’re enrolled in Original Medicare (Parts A & B)
- You live at home or in an independent community
- You are caring for someone living with dementia
