Our highly-trained social workers and case managers can provide you with the assistance you need to live safely and comfortably in your own home so you can “age in place” and avoid premature, long-term nursing home placement. North Shore Senior Center is accredited by CARF International, an independent, nonprofit accreditor of health and human services. This accreditation validates our commitment to excellence and the well-being of the older adults we serve.
We offer many in-home and community-based programs and services for people who need support and for their family caregivers.
Programs for Support at Home:
Choices for Care—Designed to ensure that individuals who are planning to enter a nursing home actually need that level of care, this program also explores alternative community resources and options so an informed decision can be made.
Chore Housekeeping—This program can help provide a trained housekeeper to assist older adults by doing light housekeeping several hours each month. An assessment is required to determine eligibility.
Community Care Program (CCP)—This program provides eligible older adults with a trained homemaker to assist with personal care and household tasks. CCP also may provide for adult day services in which the individual can spend time engaged in various supervised recreational/social activities. An Emergency Home Response System is available to those who qualify.
Friendly Visiting Program—This outreach effort allows for North Shore Senior Center volunteers and home-bound older adults to socialize on a weekly basis and forge lasting friendships.
General Case Management—This program requires an assessment to determine an individual's needs before a plan of care can be implemented. Common issues involve applying for benefits, arranging for help in the home, home-delivered meals, or gaining access to medical care and affordable housing.
Home-Delivered Meals—This program ensures that isolated older adults can enjoy healthy, nutritious food in their homes. North Shore Senior Center provides assessments to determine need, makes referrals to meal providers, and monitors the program’s effectiveness.
Lending Closet—The Lending Closet is open for loaning out and accepting returns of previously borrowed equipment as well as donations of mobility equipment. Due to health and safety concerns, the Lending Closet will no longer loan out or accept personal care/hygiene equipment such as raised toilet seats, commodes, transfer benches, shower chairs or crutches. Appointments must be made to borrow, return or donate equipment.
Escorted Transportation Service—CURRENTLY ON HOLD
Wellness Education – We provide a variety of ongoing health and wellness education programs at local senior housing buildings. Topics include arthritis, nutrition, exercise, asthma and osteoporosis.
